State of Massachusetts Licensed Practical Nurse II - PER DIEM in Holyoke, Massachusetts
THIS IS AN OPEN AND CONTINUOUS POSTING PER DIEM POSITIONS ON MULTIPLE SHIFTS.
The Soldiers' Home in Holyoke is seeking compassionate and experienced nursing professionals to apply for our Licensed Practical Nurse II (LPN) positions. Candidates will assume supervision of an assigned unit staff, while fostering teamwork across multiple disciplines. The LPN is responsible for providing patient driven care and support to a Veteran population and is expected to follow and contribute to a plan of care for individual patients in accordance with nursing standards, policies and procedures. The incumbent will work both as a team member and independently, communicating in a clear and constructive manner to members of the team, patients and their families.
Duties and Responsibilities (these duties are a general summary and not all inclusive):
Provide exceptional nursing care for veterans by assessing health status, responding to call lights or veterans' requests for assistance, and making observations on rounds;
Administer nursing treatments, procedures, doctors orders, and care plans; observe, identify and respond to the needs for care, including medication, equipment-assisted care, and patient/family education;
Identify, evaluate and assess veteran health status; confer with supervising Registered Nurse (RN) and assigned doctor for treatment modalities;
Recognize urgent or emergency patient care situations; seek assistance of the RN and/or Medical Doctor and initiate appropriate emergency interventions as directed or needed;
Collaborate with other residential services personnel to ensure that residents' individual care plans are being followed;
Establish constructive relationships with veterans and their families to promote positive relationships and learning as established by the "treatment/teaching plan";
Document incidents with or without adverse effect to the veteran and refer to RN for follow-up and/or investigation
Foster the Soldiers' Home in Holyoke mission, vision and operational policies while maintaining regulatory and department compliance with Joint Commission, and state/federal regulations;
Participate and/or facilitate orientation and instruction for other nursing personnel, including techniques, procedures and equipment;
Maintain personal and professional growth by attending in-services, keeping current with healthcare trends and nursing practices, and other Soldiers' Home committees;
Participate in daily or weekly staff meetings to discuss patient status and other concerns or issues;
Provide ongoing feedback/appraisal to assigned staff; updating supervising RN of staff performance matters via consultation and documentation.
This is a 24 hour/7 days a week staff position. The Soldiers' Home in Holyoke requires employees in this position to be able to perform mandatory overtime when circumstances dictate the need.
- Must possess a current, active license to practice as a LPN in the Commonwealth of Massachusetts
Proficient understanding of the principles and practices of geriatric nursing, dementia, hospice, and modern long-term care;
Knowledge of standard nursing policies, procedures, methods and techniques;
Capacity to maintain accurate records and exercise discretion in handling confidential information;
Familiarity with counseling techniques, safety practices, and medical terminology/abbreviations.
- Per Diem staff will be expected to be available to work a minimum of sixteen hours per month, at least eight hours of which shall be on a weekend shift, in a week which a holiday occurs, or on a holiday.

MINIMUM ENTRANCE REQUIREMENTS
Applicants must have at least one year of paraprofessional experience in the field of nursing as a licensed practical nurse.
SPECIAL REQUIREMENTS: Possession of a current and valid license as a practical nurse under the Massachusetts Board of Registration in Nursing.
