State of Massachusetts Licensed Practical Nurse I (LPN) -PER DIEM in Holyoke, Massachusetts

This is an open and continuous posting for Per Diem staff.

The Soldiers' Home in Holyoke is seeking several enthusiastic, compassionate and experienced nursing professionals to provide patient driven care and support to our Veteran population. These individuals are expected to follow and contribute to a plan of care for individual patients in accordance with nursing standards, policies and procedures. The incumbent will work both as a team member and independently, communicating in a clear and constructive manner to members of the team, patients and their families.

/DUTIES AND RESPONSIBILITIES (these duties are a general summary and not all inclusive):/

  • Provide exceptional nursing care to Veterans by assessing health status, responding to call lights or Veterans' requests for assistance, and making observations on rounds;

  • Administer nursing treatments, procedures, doctor orders, and care plans; observe, identify and respond to the needs for care, including medication, equipment-assisted care and patient/family education;

  • Identify, evaluate and assess Veteran health status; confer with supervising Registered Nurse (RN) and assigned doctor for treatment modalities;

  • Recognize urgent or emergency patient care situations; seek assistance of the RN and/or Medical Doctor and initiate appropriate emergency interventions as directed or needed;

  • Collaborate with other resident services personnel to ensure that residents; individual care plans are being followed;

  • Establish constructive relationships with Veterans and their families to promote positive relationships and learning as established by the "treatment/teaching plan";

  • Document incidents with or without adverse effect to the Veteran and refer to RN for follow-up and/or investigation;

  • Foster the Soldiers' Home in Holyoke mission, vision and operational policies while maintaining regulatory and department compliance with Joint Commission, and state/federal regulations;

  • Participate and/or facilitate in orientation and instruction for other nursing personnel, including techniques, procedures and equipment;

  • Maintain personal and professional growth by attending in-services, keeping current with healthcare trends and nursing practices, and other Soldiers' Home committees;

  • Participate in daily or weekly staff meetings to discuss patient status and other concerns or issues;

  • Provide ongoing feedback/appraisal to assigned staff; update supervising RN of staff performance matters via consultation and documentation.

/REQUIREMENT:/Must possess a current, active license to practice as an LPN In the Commonwealth of Massachusetts;

/PREFERRED QUALIFICATIONS:/

  • Proficient understanding of the principles and practices of geriatric nursing, dementia, hospice, and modern long-term care;

  • Knowledge of standard nursing policies, procedures, methods and techniques;

  • Capacity to maintain accurate records and exercise discretion in handling confidential information;

  • Familiarity with counseling techniques, safety practices, medical terminology/abbreviations.

/SCHEDULE:/

  • Per Diem staff will be expected to be available to work a minimum of sixteen hours per month, at least eight hours of which shall be on a weekend shift, in a week which a holiday occurs, or on a holiday.

/PREFERRED QUALIFICATIONS:/

  • At least one (1) year of experience in the care of geriatric patients in a healthcare setting such as long-term care;

  • Certification in dementia, hospice or other relevant discipline;

  • CPR certification through the American Heart Association;

  • Ability to establish effective relationships with individuals ,families, and team members;

  • Demonstrated attention to detail with readiness to respond appropriately the needed;

  • Capacity to maintain accurate records and exercise discretion in handling confidential information;

  • Skilled in the use of using a computer for completion of work assignments.

The Soldiers' Home in Holyoke is a state-funded, fully accredited health care facility that offers Veterans quality health care, including full-time residential accommodations, and on-site dental clinic, a multi-service outpatient department and provides education and social services for approximately 5,000 Veterans. The Soldiers' Home has a total of 248 long term care beds, 30 residential beds, and is staffed by approximately 350 administrators, clinicians, nurses, certified nursing assistants, skilled trade, dietary staff, housekeeping staff, contractors, consultants and general administrative personnel.
