Hospice RN Case Manager Holyoke VNA & Hospice ? HVNA Clinical Holyoke, MA NursingFull Time, 40, varying shifts Req # 6658 The Holyoke Visiting Nurse Association, Inc., established in 1905, is dedicated to providing quality skilled care to individuals in the comfort of their home. We coordinate and deliver comprehensive, cost-effective services including nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide care. In addition, we are committed to promoting community health through education and special programs. The Holyoke Visiting Nurses Association, Inc. is looking to hire a Hospice Registered Nurse working either 30 or 40 hours on our Hospice Team. This position is eligible for a retention bonus of up to $10,000!! The Hospice RN Case Manager is responsible for: The design and implementation of the plan of care, and initiating changes in the plan of care when appropriate. Evaluation and improvement of clinical and financial patient outcomes. Proactively facilitating coordination of services and utilization of community resources. Coordinating care and communication between team members and physicians to ensure appropriateness of care. Planing and facilitating the appropriate discharge from services. Providing skilled nursing care to patients in the community and inpatient setting in accordance with agency policies and procedures. Providing care in a manner, which promotes quality, continuity and safety for the patient. Providing professional nursing care utilizing all elements of the nursing process. Assessing patient?s physical and emotional needs. Maintaining a working knowledge of community resources. Responsible for ongoing evaluation of patient?s progress towards goals. The completion and maintenance of accurate and timely documentation and relevant clinical notes regarding the client?s condition and all care & treatment given. Supervision of home health aides and delegate responsibilities Minimum Requirements: Registered Nurse (RN) Licensed in state of MA CPR Certification Demonstrated clinical & computer skills Home care experience - preferred
