Advocates Residential Nurse - Developmental Services in Framingham, Massachusetts

Overview

$35/hour

The Registered Nurse provides nursing services as defined in the RN scope of practice and under the direction of the Director of Nursing.

Minimum Education Required

Bachelor's Degree

Responsibilities

  1. Responsible for continuous review of all aspects of caseload to include review of documentation in medical record, maintenance of interdisciplinary team communication and collaborative development of plans of care with the team.

  2. Provide on-site support to direct care staff and program managers by assisting in the development of strategies to address the special needs of individuals with a variety of acute and chronic health issues.

  3. Communicate and network with other health care professionals and providers within program communities to develop relationships and improve the care individuals receive from external providers.

  4. Assist to streamline communications between agency and the DDS regarding health and safety issues by:

  5. Maintaining in-house record keeping and reporting of incidents, behavioral episodes, and any other day to day monitoring of acute and chronic health related issues.

  6. Provide as needed instructions on how to put together the “personal well being” piece of the ISP for individuals with complex health issues.

  7. Teach staff who attend ISP meetings on how to present medical data and to communicate effectively with DDS service coordinators.

  8. Provide on call assistance to program staff regarding any medication related issues.

  9. Provide and collaborate with program director, follow-up instructions to program staff involved in medication occurrences.

  10. Teach vital signs certifications, health protocols, and any other DPH/MAP approved classes.

  11. Follow up on DPH registered programs and their med certified staff to ensure that MAP guidelines for the administration, documentation, storage, and disposal of medications are being followed.

  12. Seek to ensure the continuity of care by avoiding fragmentation or duplication of services.

  13. Monitor and evaluate individuals care and generate ideas for quality improvement.

  14. Attend, as needed, post hospitalization discharge meetings.

  15. Attend trainings as assigned; maintains necessary certifications (CPR, MA LPN).

  16. Strictly adhere to all HIPAA regulations and always maintain confidentiality

  17. Oversee the direct care of individuals with complex health issues.

  18. Provide direct nursing care only in special circumstances and for a limited amount of time when attempts to engage VNA or other generic health care systems have been exhausted.

  19. Adhere to all principles related to the Advocates Way.

  20. Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights policy.

  21. Attends and actively participates in supervision and staff meetings.

  22. Perform all duties in accordance with the agency’s policies and procedures.

Qualifications

  1. B.S in Nursing and must possess a current, state of MA, registration in nursing.

  2. One year of experience in a community health care setting is preferred.

  3. Demonstrate an interest in working with the disabled population.

  4. Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.

  5. Must be able to perform each essential duty satisfactorily.

  6. Must have sensitivity to the needs of the population.

  7. Strong computer knowledge.

  8. Excellent organizational, time management and problem-solving skills

  9. High energy level, superior interpersonal skills and ability to function in a team atmosphere.

  10. Excellent communication skills, written and verbal.

  11. Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
