Making Opportunity Count Inc Psychiatric Nurse Practitioner in Fitchburg, Massachusetts
BASIC FUNCTION: Provide clinical care as part of an outpatient mental health clinic. Primary responsibilities include the evaluation and treatment of children, adolescents, and adults with a broad spectrum of mental health challenges.
o Works collaboratively and cooperatively with other professionals, consulting with, and to, other providers to optimize clinical outcomes. Establishes and maintains professional relationships with individuals served and their families. Attends and participates in treatment team meetings, and in departmental and divisional meetings, as assigned.
o Conducts clinical assessments for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities, inclusive of obtaining health history through interviews of individuals-served and their families, clinical record review, and performing appropriate physical examination. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screening, immunizations and patient care. Refers to other healthcare providers, as appropriate.
o Participates in the development of appropriate, evidence- and assessment-based, person-centered treatment plans in consultation with individuals-served, their families, and other team members. Educates individuals-served and families regarding medications and treatments, and about health promotion/illness prevention. Recommends appropriate community resources to meet the needs of individuals-served and their families; and communicates relevant case management information to other professionals and community providers.
o Provides interventions, including direct treatment and management of health conditions via prescriptive authority, in compliance with medical practice guidelines and state laws. Completes documentation in the electronic health record related to interaction with individuals-served, families and other service providers in an accurate and timely manner, and in accordance with professional ethics and with both Federal and State regulations.
o Reviews clinical records to determine the progress, effectiveness, and appropriateness of treatment-team services offered. Participates in quality improvement and initiatives that analyze professional practice, outcomes and efficiencies, including peer review, chart review, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures.
o Other related duties as assigned.
o Master’s or Doctoral degree. Currently licensed as an Advanced Practice Nurse-Psychiatric Mental Health Nurse Practitioner in the Commonwealth of Massachusetts and in good standing.
o Graduate of licensed and accredited school of nursing.
o Eligible for credentialing and privileging.
o Excellent written and verbal communication skills.
o Outstanding organizational and time-management skills.
o Ability to address individuals and information with a high degree of sensitivity and confidentiality in various delicate, formal, or urgent situations.
o Ability to develop and maintain effective working relationships with physicians, staff, and patients.
o Must be comfortable and flexible with changing priorities and self-directed problem solving.
o Must have acceptable background check.
o Must have a valid driver’s license and the ability to drive a vehicle as needed.
o Three or more years of experience in a busy outpatient clinical practice with minimal clinical guidance.
o Experience delivering care through a multidisciplinary team.
o Proficiency using electronic medical records.
o Experience tracking, measuring, and using patient reported outcomes in developing treatment plans for patients.
o Bilingual in English and Spanish.
