The Medically Assisted Treatment Medical Assistant-Case Manager provides administrative support with program data collection, maintenance and reporting. The Medical Assistant provides assistance to the patients and providers to manage the flow of program visits and services across CHC. The Medical Assistant will assist with data collection and entering into the appropriate systems, and that follow up with patients to coordinate appointments is completed. The position works in conjunction with the RN-OBOT and the Nurse Manager-Fitchburg, and demonstrates excellent decision-making skill, problem solving, and attention to detail. The position is responsible for highly sensitive information, which requires a clear understanding of medical and behavioral health confidentiality.
Responsibilities include:
Prepares patients for visits with providers by performing and recording vital signs (including: blood pressure, temperature, pulse, respirations, weight, and other procedures as needed)
Assists in preparing patients for routine diagnostic procedures (i.e. urine specimens) and performing selected POC screening tests
Work is of medium demand, walking and standing.
Managing patient care is a team effort that involves clinical and non-clinical staff interacting with patients
Organization and coordination of Suboxone and Vivitrol schedules, distribution of patients across sites and provider time blocks.
Management of provider registry for patients receiving MAT
Provide support during screening, intake, induction, stabilization and maintenance phases of patient treatment
Tracking, follow-up and reporting of diagnostic services, diagnostic test results, referrals for diagnostic tests, tracking of results and closing the loop for all test results
Communication with providers, patients and the patient’s families when appropriate
Assists with maintaining the treatment database with health information for those patients enrolled in the program and pertinent to any associated grants
Required to attend mandatory program meetings and trainings
Works independently on special nonrecurring and ongoing projects as needed
Demonstrate understanding and commitment of the health center mission
Demonstrate understanding and commitment to the established CHC Values and Standards
Performs other job related duties as required or assigned
Minimum Qualifications:
High School Diploma or GED required, Associate Degree in Medical Office Assistant or equivalent preferred
Previous experience preferred
Work experience similar to a medical office environment
Computer skills for accurate data entry
Knowledge of basic medical terminology
AHA BLS Health Care Provider certification or equivalent
Demonstrated interpersonal relationship skills
Demonstrated written and verbal communication skills in English
Demonstrated ability to work in a fast paced medical office environment
