Position Overview:
The Community Health Outreach Worker is responsible for helping patients and their families understand and access reproductive health services provided at M.O.C.’s reproductive health clinic. Within an integrated approach, the CHOW is responsible for leading all outreach efforts in the community, educating and encouraging access to health services to a diverse population. The CHOW will have a special focus in engaging youth and women of diverse backgrounds and offer culturally sensitive family planning supports that are reverential to these groups.
Essential Duties and Responsibilities:
Knowledge of M.O.C.’s catchment of communities, local organizations, programs, and providers that serve our target population including M.O.C.’s own services.
Develops and implements an aggressive outreach plan that targets youth and women with an emphasis on Spanish and Portuguese speaking populations.
Responsible for establishing trusting relationships with patients and their families while providing general education, support and encouragement.
Identifies, engages with and attends community meetings representing at a minimum youth and women of diverse backgrounds to help them understand the scope of reproductive services offered by M.O.C.’s Reproductive Health Clinic.
Provides ongoing follow-up, basic motivational interviewing and goal setting with patients/families.
Follows up with patients via phone calls, home visits and visits to other settings where patients can be found.
Assist patients set personal health related goals and attend appointments, provide referrals for health-related services, connect patients to transportation resources and provide appointment reminders in special circumstances.
Exhibits excellent working relations with patients, clinic visitors and staff.
Works closely with medical providers to help ensure that patients have comprehensive and coordinated care plans. Assist patients with follow-up appointments to related reproductive medical services in the community as needed.
Records patient care management information in the EMR and other software no later than 24 hours after patient contact.
Maintain HIPPA compliance at all times.
Required Qualifications and Skills:
Written and oral fluency in English, matching fluency in Spanish/Portuguese preferred.
Experience working in a multi-cultural setting.
Experience working in a community-based setting for at least 1 to 2 years.
Experience in community outreach planning and implementation.
Excellent communication skills and comfortable public speaking to a wide audience.
Basic computer skills required; electronic medical record (EMR) experience preferred
Familiarity with local communities served by M.O.C.
Ability and willingness to provide emotional support, encouragement and motivation to patients.
Ability to forge relationships with community organizations and groups.
Solid organizational skills to handle multiple priorities while remaining professional and calm.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift up to 15 pounds at times.

Compensation:
A salary within the range of $20.43 to $27.64 per hour will be provided to the successful candidate having met the minimum requirements for the position consistent with the MOC compensation philosophy.
