Family Service Association - Fall River Licensed Practical Nurse (LPN) in FALL RIVER, Massachusetts
$1000 sign on bonus!
We are currently seeking 1 part time (30 hours per week) LPN at our Bridgeview Adult Day Health program in Fall River, MA. The hours are Monday through Friday 8:00 a.m.- 2:00 p.m.
The ideal candidate will assist the RN as liaison between participant's primary care physician and will monitor participant's health status and care plan. Some of the responsibilities are to complete medical assessment forms, provide case management services as designated by RN; review and supervise participant's personal care services; attend monthly supervisory meetings with RN; writing monthly nursing progress notes in each participant's record; and assist the Program Manager with the implementation of policies and procedures.
Education and Requirements:
Must be currently licensed in the Commonwealth of Massachusetts and in good standing.
C.P.R. Certified
Two years experience working with aging and disabled population strongly preferred.
An ability to work with others as part of a team and communicate effectively.
Must be able to successfully complete a MA State CORI check, CPS check and company paid, and pre-employment physical.
Bi-lingual English/Spanish and English/Portuguese speaking applicants are encouraged to apply.
