State of Massachusetts Coordinated Specialty Care Coordinator (HSC I) in Fall River, Massachusetts

The Massachusetts Department of Mental Health (DMH) is seeking a compassionate and dynamic candidate to serve as the primary case manager for young adults enrolled in the Coordinated Specialty Care (CSC) program.The CSC program is an exciting new program primarily serving the Greater Fall River area to meet the needs of young adults and families experiencing First Episode Psychosis. The Case Coordinator will receive specialized training including Navigate to enhance skills to provide evidence-based individual and family care coordination services to assigned persons served. _Please note_: this position is full time 40 hour per week. 8:30am – 5:00pm (will work one evening shift a week) Days off: Sunday and Monday The selected applicant will provide care coordination services to assigned persons served, family support, in line with the CSC model and serve as a member of the integrated care team. This position will lead trauma-informed, person centered assessment and treatment planning activities and ensuring the interventions are delivered in accordance with the CSC model and utilizing evidence-based interventions &/or best practices. _Duties and Responsibilities (these duties are a general summary and not all inclusive):_ * Supports immediate and concrete needs to persons served regarding housing, income, criminal justice needs and transportation. * Provides education about, and link to, available community resources for both children and adults. * Offers care coordination to assigned caseload, including providing referrals for any life domain impacting overall health and wellness. * Employs interventions appropriate for current presentation and stage of treatment. * Supports client and family access to age-appropriate educational and community resources. * Ensures timely documentation of person served progress and to ensure compliance with existing DMH regulations and program procedures. * Collaborates and intervenes with families using evidence-based practices including assertively in times of heightened risk or crisis. * Coordinates hospital discharges and support through care transitions. * Provides life skills training, through use of individual coaching and group modalities. * Attends weekly CSC team meetings and daily rounds. * Ensures integration with Care Coordination entities such as DMH targeted Case Management, BHCP, and OneCare. /_Preferred Qualifications:_/ * Current and valid MA Class D Motor Vehicle Operator’s license or the equivalent from another state is necessary for this funciton * Professional experience providing care coordination services to young adults with behavioral healthcare needs * Given the population served, bilingual or multilingual fluency. /_DMH Mission Statement:_/ The Department of Mental Health assures and provides access to services and supports to meet the mental health needs of individuals of all ages, enabling them to live, work and participate in their communities.
