Eliot’s Certified Community Behavioral Health Clinics (CCBHC) in Everett and Lynn are seeking Licensed Practical Nurses with at least two years experience working with individuals with substance use and mental illness.

The goal of Eliot’s CCBHC is to expand the impact of evidence-based treatments for underserved populations in the Everett and Lynn areas. Eliot’s CCBHC is a team-based model of care that includes psychiatrists, nurse practitioners, nurses, case managers, triage specialists, recovery coaches, and peer specialists.

The ideal candidate will be bilingual, passionate about working within the behavioral health field, able to work in a fast-paced environment, and skills in data management.

Responsibilities:

  • Provide education and consultation to individuals and staff regarding medical health, medication and medical topics.

  • Provide at least quarterly medical screening of all assigned individuals. Obtain individual weight, height, BMI and other aspects related to metabolic syndrome to establish a baseline.

  • Identify individual preference for making changes as applicable in regarding medical conditions, nutrition, smoking cessation, exercise or other areas related to overall health, utilizing motivational interviewing techniques.

  • Provide side by side interventions to individuals that focus on problem solving, skills training, modeling behavior, optimism and encouragement.

  • Participate in daily Team Huddles and Team Meetings.

  • Assist individuals with accessing community resources that will support and assist with addressing needs and health wellness efforts.

  • Provide oversight, guidance and assistance to individuals with understanding their medical conditions. Meet with individuals and staff to discuss any medical diagnoses and conditions, causes, symptoms, complications, and treatment options.

  • Perform direct medical care for individuals as appropriate and ordered by physician which may include monitoring of vitals, wound care, etc. This includes providing interventions specific to individual treatment plan for medical/health and wellness goal areas.

  • Provide assistance coordinating overall medical care for individuals and ensuring medical follow up is done as needed.

  • Develop working relationships with community medical providers and VNA related services in order to assist with coordinating medical care for individuals on team. Assist with acting as a liaison to communicate medical issues and concerns to medical providers and ensuring the treatment team has an understanding of concerns.

  • Assist staff with obtaining VNA services, having a individual leveled for nursing care or obtaining a referral for a PCA or other related service as needed. Ensure that all prior to a leveling, the treatment team has assisted individual with accessing any type of services that would allow them to remain independently in the community, if appropriate or possible.

  • Participate in on-call on a rotating basis with the entire CCBHC team.

Qualifications:

  • Must be a licensed practical nurse and have at least two years of experience working with people with severe and persistent mental illnesses.

  • Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses and respect for their preferences for treatment.

  • Must have the ability to work on a team that provides flexible community based services.

  • Computer literacy and skill in the use of computers and related software applications.

  • Ability to consistently meet all necessary background and credentialing requirements.

  • Must have valid drivers license and reliable transportation. Must have reliable telephone notification system for emergency contact.

Schedule: M-F 9-5

Pay differential for bilingual candidates.

Sign-On Bonus Payout:

500 after NSO

500 after 30 days employment

1000 after 6 months of employment and 1000 after 1 year of employment.
