Provides episodic nursing care & assistance with other residential tasks, as needed. Consults with residential staff regarding health issues of individuals in accordance with the Department of Developmental Services (DDS), Medication Administration Program (MAP) & Agency policies. Provides direct oversight & training of staff on shift in meeting individual needs. ESSENTIAL JOB FUNCTIONS In collaboration with medical team, consults on individual care. Contributes to the development & implementation of residential assessment within 120 days of intake & individual service plan (ISP) guidelines thereafter. Sets measurable objectives for individualized care plans to meet individual?s physical, mental & psychosocial needs. Assesses need for preventative & restorative programs to ensure optimal quality of life for each individual. Assists in reviewing individual?s health care records & tracks appointments in accordance with DDS regulatory requirements & recommendations. Maintains appointment tracking records. Reviews individual logs & verbal staff reports, communications & residential medical records for health issues & the tracking of current, ongoing health concerns; communicates issues to Registered Nurse & Residential Supervisor, as needed. Provides episodic nursing care & assistance to individuals. Administers medications, treatments, & skilled nursing care in accordance with DPH Medication Administration Policies & Procedures & according to health care practitioner orders. Adheres to infection control procedures. Monitors residents? positioning & use of supports. Assesses residents? skin integrity, monthly or more frequently as HCP recommended. Assesses functional status. Develops accurate Master Physician?s Orders, verified in partnership with the Residential Supervisor, & insures review & signature by the Primary Health Care Practitioner. Assists in conducting weekly medication ordering & maintenance of medications logs in accordance with MAP regulations; reviews medication supplies for suitability to MAP standards. Attends medical appointments, as assigned by management, & ensures health care practitioners have required documentation. Adheres to established lines of communication; represents a positive & pro-active attitude with staff, families, supervisors & DDS representatives. Provides training to direct care staff, as needed. Provides individuals, families & staff with education & information, as needed. Attends all required meetings & trainings. Adheres to Agency emergency response policies & procedures. Serves as mandated reporter of suspected cases of abuse &/or neglect to the Disabled Persons Protection Commission (DPPC) or Elder Services. OTHER DUTIES & RESPONSIBILITIES Reviews & maintains first aid supplies inventory on a monthly basis. Provides assistance to staff with client ADL?s, including but not limited to, medication & dietary management, personal hygiene, & other activities, as needed. Transports individuals to appointments & social activities utilizing the Agency-assigned vehicle. Performs additional duties, as assigned.
