Brockton Hospital Physical Therapist - EB YMCA in East Bridgewater, Massachusetts

Position Summary:

Under the supervision of the Coordinators of Rehabilitation Services, evaluates plans and provides direct patient care in accordance with physician orders to rehabilitate therapeutically the physical faculties of outpatients, inpatients, and Transitional Care Unit patients in order to achieve stated goals. Services are provided according to hospital and departmental policies and procedures and in compliance with regulatory standards.

This is a full-time flex position

Responsibilities:

  • Completes a full initial evaluation on each patient referred appropriate to patient's diagnosis according to established procedure and physician order.

  • Provides timely documentation at the time of evaluation

  • Provides clear daily and progress reporting

  • Instructs and assesses patients in a variety of functional activities.

  • Performs and assists patients through a variety of therapeutic modalities and techniques per established procedures, utilizing techniques specific to age group serviced.

  • Reassess and recommends modification of treatment plan and goals. Responds to changes in the physical and mental status of patients and to any duration in the anticipated rate of progress.

  • Meets with the members of the patients care team in development and implementation of assessments, care plans and discharge planning.

  • Instructs patient care staff in therapeutic techniques and equipment to facilitate the carryover of therapy plan in daily activities.

  • Delegates treatments to PTA’s to improve flow and productivity

  • Provides ongoing patient, family and caregiver education.

  • Presents with a positive attitude and demeanor at all times, adheres to the WE CARE standards

  • Proficient in the use of computerized documentation

  • Utilizes appropriate outcome measures

  • Participates in departmental Lean activities

  • Participates in (1) departmental projects/initiatives / year

  • Provides supervision to Physical Therapy Assistants, and rehabilitation aides.

  • Provides clinical supervision to affiliating students under guidelines established by the participating educational institutions.

  • May participate in the mentoring program.

  • Establishes daily schedule, treatment plans, and priorities to service assigned patient.

  • Maintain established policies and procedures, quality assurance, safety and infection control.

      1. Communicates effectively with internal and external customers with respect of differences in cultures, values, beliefs and ages, utilizing interpreters when needed.

    Performs other duties as assigned

    BASIC KNOWLEDGE/SKILLS/APTITUDE/EXPERIENCE:

  • Establishes daily schedule, treatment plans, and priorities to service assigned patient.

  • Generally works independently within department policies however, will consult coordinators regarding unusual problems or cases.

  • Maintain established policies and procedures, quality assurance, safety and infection control.

  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

    Education/Experience/Licenses/Technical/Other:

  • Education: Bachelor in Science in Physical Therapy Experience (Type & Length): 5 years clinical experience or Masters of Science in Physical Therapy . Experience (Type & Length): Previous experience is not required outside of clinical requirement.

  • Experience: Requires a minimum of one year on the job to be proficient in duties. Basic CPR training will be provided.

  • Certification/Licensure: Registration/Licensing in the State of Massachusetts required.
