Description:

A licensed practical nurse bears full responsibility for the quality of health care she/he provides, including participation in direct and indirect nursing care, health maintenance, teaching, counseling, collaborative planning and rehabilitation.

Excellence in nursing practice also includes clinical skills and capabilities beyond baseline competence. These include the ability, in collaboration with the clinical team, to conduct a comprehensive assessment that incorporates an advanced and sophisticated integration of clinical information, and the ability to translate that assessment into a superior plan of care for the patient/family/care team.

In addition, excellence in this position includes global skills and capabilities beyond baseline competence. These include: demonstrating a comprehensive understanding of the department’s immediate needs and long term growth goals, showing initiative, using critical thinking to problem solve independently, exhibiting leadership qualities, and effectively engaging with the clinical team. provide direct patient care, patient education and triage. Emergency room experience required as well as previous ambulatory care experience. Must have MA LPN License.

Schedule is 20 hours/week consisting of day/evening/overnight rotating and will include weekend/holiday rotation subject to the needs of the department.

Essential Duties & Responsibilities:

  • Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.

  • Provides thorough and concise documentation of patient’s histories, assessments, plans, and treatments. Enters messages as staff messages or telephone encounters in the computer. Enters and views patient letters in the computer. Enters exam encounters in the computer.

  • Manages all aspects of the communication in-basket including response, results, telephone encounters etc. Attains and maintains proficiency in the use of the electronic medical record as it pertains to the workflows of the staff nurse.

  • Follows protocols for management of patients with chronic/acute disease and health promotion.

  • Follows established departmental policies and procedures for environmental safety and infection control. Actively participates in EBNHC programs and committees.

Knowledge, Skills, and Abilities Required:

  • Current ACLS and PALS Certification required.

  • Bilingual English/Spanish preferred.

  • 1 year relevant nursing experience, preferably in a community health and/or ambulatory care setting.

  • High School Diploma. Completion of vocational/technical training beyond high school in an Accredited Nursing Program. Licensure in Massachusetts as an L.P.N.

  • Demonstrated ability to communicate effectively both in writing and verbally. Familiarity with the use of a personal computer in a Windows environment preferred.
