Job Title: Behavioral Health Registered Nurse (RN)
Location: Webster/Southbridge
Work Schedule: Monday through Friday (Day Shift)

Your Role & Responsibilities:
Holistic Assessment: Conduct thorough assessments to identify the needs of behavioral health patients and their families.
Personalized Care: Manage and deliver comprehensive care from initial assessment to follow-up evaluation.
Team Collaboration: Communicate effectively with the clinical team to develop and implement care plans that optimize patient outcomes.
Patient & Family Education: Educate patients and their families about behavioral health principles, symptom management, and care plans.
Quality Compliance: Ensure all care provided meets Elara Caring’s policies, procedures, and quality standards.
What We’re Looking For:
Licensed RN: Current, unrestricted RN license in the state.
Clinical Experience: Minimum of 1 year of experience in a clinical care setting, with a preference for behavioral health.
Mobility: Ability to travel within the designated branch/office coverage area (50% of the time).
Reliable Transportation: Must have a dependable vehicle, valid driver’s license, and current auto insurance.
Educational Background: An Associate or Bachelor’s Degree in Nursing is preferred.
Preferred Experience: Previous experience in Home Health or Hospice and familiarity with Homecare HomeBase is a plus.
