Transitions of Care Manager

The Transitions of Care Manager is responsible for overseeing the efforts to develop a program-wide coordinated response to transitions of care. This position utilizes research findings in practice and participates in Transitional Care program design, implementation, and evaluation and participates in ongoing quality improvement activities. He or she will work with Affiliated Partners to understand barriers and challenges in transitions of care efforts and identify, design, and implement potential program-wide workflows to improve Behavioral Health Community Partner's ability to support our members during and after the discharge process.

While the qualifications include a Nursing Degree, clinical care is not included in the responsibilities of this role; rather, the Transitions of Care Manager will focus on relationship development and operational improvements to help improve quality of care for Behavioral Health Community Partner members and support the program in achieving quality benchmarks.

Required Skills

  • Excellent written and verbal skills in the English language required

  • Valid driver’s license and regular access to a reliable vehicle required

  • Basic computer fluency, including Microsoft Office, required

Required Experience

  • Bachelor of Science degree in Nursing required

  • Minimum of four years of nursing experience required, with two years in community health and two years of nursing case management preferred

  • Knowledge of the needs of individuals with behavioral health conditions preferred
