Coordinates and responds and processes requests for medical information/records.

Job Description:

Review requests and identify potential medical/legal cases and notify the Department of Health Care Quality or the Legal department.

Respond to Correspondence requests to include acceptance of subpoena/court orders and recording the acceptance into the department Subpoena Log.

Screen records consistently for confidential information that is not covered under a general authorization.

Reviews all authorizations and check the validity according to hospital policy. Discuss with requester (patients, physicians, government and regulatory agencies, etc.) the proper procedures for obtaining copies of medical records and/or completed medical forms.

Work with requester to secure the appropriate consent. Answer questions posed by the requester regarding status of requests for patient information.

Direct patients/requesters to specific clinics, ancillary departments, physician office, Radiology, Billing when further medical information is needed that can be provided elsewhere as well as Record requests for Correspondence area.

Certify copies of medical records, as delegated by manager, Photocopy requested data and forward to appropriate parties.

Discloses information in compliance with center policies, Massachusetts' General laws and Federal Public Health laws.

Preferred Qualification:

*High school diploma or equivalent

*Medical Terminology (preferred)

*Epic experience preferred

*Must be detailed orientated, able to multitask and excellent customer service
