Acadia Healthcare Case Manager Per Diem in Dartmouth, Massachusetts

Description

The Case Management position is responsible for providing quality care to adolescent, adult and geriatric patients, and their families and guardians throughout the patients’ episode of care. Quality care includes, but is not limited to the following: performing psychosocial assessments, participating in multi-disciplinary rounds and treatment planning meetings, providing care coordination, organizing individual/family/systems meetings, facilitating clinical process groups, managing the discharge and continued care planning process and documenting all patient encounters.

-Review of patient's clinical documentation from referring facilities

-Conduct individual sessions with all clients minimally one-time weekly

-Complete timely and thorough documentation:

  • Psychosocial Assessment completed with 48 hours of patient's admission

  • Outreach and care coordination with patient's providers within 48 hours

  • Initial Multidisciplinary Treatment Plan completed within 72 hours of patient's admission

  • Treatment Planning updates completed weekly

  • Progress notes completed using the DAP format for each patient encounter

  • Discharge Plan created with patient participation

  • Discharge note including discharge and care coordination plan

  • Release of information completed prior to coordinating care and contacting collaterals

-Participate in weekly Case Management Departmental Meetings

-Participate in process/quality improvement measures

-Facilitate process groups

-Attend clinical trainings and conferences to complete required CEUs and to increase skills/knowledge base

-Complete required CPI, CPR and other orientation and annual hospital trainings

-Maintain appropriate state licensure

Two to Five Years of Professional experience in settings with acute psychiatric and substance abuse disorder patients(adolescent, adult and geriatric)

Independently Licensed or Masters Prepared Licensed eligible in Massachusetts

Experience with evidence-based practice preferred
