This is a 30 hour/week Ambulatory Clinical Pharmacist for Anticoagulation Management Services. Experience is preferred but not required.

Job Description:

Assumes responsibility to provide quality patient education. Assesses, plans, implements and evaluates patient teaching plans. Provides outpatient services including individual and group counseling sessions related to Anticoagulation medication therapy. Works collaboratively with patients, physicians, staff and other departments to achieve patient goals. Provides guidance and support for dosing and bridging patients per hospital Guidelines. Is responsible for developing and promoting collaborative and collegial relationships with nurses, physicians and interdepartmental personnel as they relate to the pharmaceutical care of the patients.
