Sugar Hill Assisted Living LPN/RN in Dalton, Massachusetts

Practices according to the philosophy, policies, and procedures of the Wellness Center under the direction of the Wellness Director. The staff LPN/RN assumes the responsibility of charge nurse assigned supervisors and assigns/directs staff according to their job description QUALIFICATIONS Must be Licensed Practical Nurse or Registered Nurse with current state license Must demonstrate competency in the following areas; Supervising Staff Assessing health care needs Assessing personal care needs Establishing and implementing plans to meet needs Communication skills, both written and oral Interpersonal relations PERFORMANCE RESPONSIBILITIES Scheduling and supervision of the assisted living community's personal care service providers Assisting in the implementation of and/or providing additional information for the service plans and updating plans as needed on an ongoing basis Assisting care providers with the implementation of service plans Overseeing the provision of assistance with activities of daily living to residents whose service plan indicate the need Overseeing self-administering medication management for residents whose service plan indicates the need Communicating pertinent information to co-workers when leaving the unit and/or when shift changes Assuming the responsibility for quality resident care and seeking guidance from supervisor personnel when needed Communication with physicians regarding resident's health and personal care needs when necessary and appropriate Determining when a resident requires meals to be served in their apartment due to illness Recording delivery of personal care services that are billable Scheduling lab tests or diagnostic tests ordered by physicians regarding resident's health Scheduling lab tests or diagnostic tests ordered by physicians, scheduling physician appointments, physical therapy, dentist, eye doctors and podiatrist visits when requested by residents or their legal representatives Visiting residents who are ill in their apartment, assess the follow up and arrange for health aids when requested and necessary Responding to emergency calls from residents immediately when on duty Welcome new resident and orienting the to the community and personal care services available at the assisted living community Evaluating resident's appropriateness for the assisted living residence regarding resident safety and comfort in an assisted living residence and communicate with residents, family and supervisor as necessary. Attend scheduled education in-services Other duties as deemed necessary
