Emerson Hospital Home Care Physical Therapist; per diem in Concord, Massachusetts
Job Summary Reporting to the Rehabilitation Supervisor, Home Care this position provides evaluation, development of individualized care planning, instruction to patient/family, and direct physical therapy services to patients/families in the home setting.
Minimum Qualifications Education Bachelor of Science in physical therapy from an accredited program.
Experience Prior Home Care experience is strongly recommended, or at least one year previous experience in an acute and/or rehab setting
Licensure and/or Certification MA PT license and BLS for healthcare provider.
