Position Title: Registered Nurse, Baylor
FLSA STATUS: Exempt
REPORTS TO: Clinical Manager
SUPERVISES: None

POSITION SUMMARY :
Therapeutic Sat-Sun 8am -8pm Work 24 hours paid for 32.
The Registered Nurse Baylor plans and provides individualized nursing care and comprehensive services to hospice patients. The Registered Nurse Baylor carries out expected duties as assigned by clinical leadership based on patient need. The Registered Nurse Baylor is expected to be available to see patients beginning at 5:00 PM on Friday and continuing through until 8:30 AM on Monday and will be paid for 40 hours per week.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :
Assesses patient’s health problems and needs and develops plan of care and updates to the plan of care.
Provides direct hospice nursing care as per physician orders.
Provides appropriate pain/symptom management. Evaluates and communicates patient’s response to treatments and medication.
Provides patient and family/caregiver education and information pertinent to patient diagnoses and plan of care as initiated by the Registered Nurse.
Maintains efficient and effective use of equipment and supplies.
Observes patients, perform comprehensive and clear documentation and reports changes in patient’s conditions, while taking appropriate and necessary actions.
Documents in medical record and prepares other reports as needed
Participates in coordination of Overlook Hospice Services, appropriately reporting the identified needs to the Interdisciplinary Team members. Members include Registered Nurse, Physician, Nurse Practitioner, Medical Social Worker, Chaplain, Certified Nurse’s Aide and Clinical Manager.
Provide informational session regarding hospice care to patients and families and admission to hospice services if appropriate.
Performs RN pronouncement of death at time of patient’s passing, completes appropriate documentation and notification per MD order.
Manages visit schedules efficiently, able to prioritize by patient acuity.
Maintain a positive culture that is built upon commitment to patients, families and other team members.
Demonstrate uncompromising respect for patients, families and team members.
Adheres to all Overlook and Hospice policies and procedures
Participates in agency quality management and safety activities
Maintain required licensure, certifications and vaccinations
QUALIFICATIONS :
Current license to practice as a registered nurse in the state of Massachusetts
Current CPR required
Three years nursing experience, preferably in hospice
Excellent nursing assessment and problem solving skills
Excellent interpersonal communication skills, both oral and written
Exceptional customer service
Strong clinical assessment, critical thinking, problem solving and organizational skills
Ability to utilize independent judgment and to follow through
Current State of Residence driver's license and reliable transportation
Computer proficiency
Ability to read, write and speak the English language
PHYSICAL REQUIREMENTS/WORKING CONDITIONS :
Primary work will be performed in the patient’s place of residence, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 25 pounds.
Required to travel to multiple work sites.
