BILH is looking for an Advanced Practice Provider (NP/PA) to join our Hospital at Home program

Job Description:

The Advanced Practitioner (AP) is responsible for providing high-quality patient care to inpatients cared for by the Hospital at Home team, as well as those being considered for hospitalization in this care model. The AP serves as the program’s representative to the patients and family in person, collaborating with the attending physician in the Command Center. The AP performs the initial face-to-face evaluation and assessment of potential patient admissions, and upon admission, the AP will collaborate with the attending physician to develop and implement an acute plan of care in the home.

The ideal candidate will have:

  • At least two years of inpatient AP experience

  • Flexibility and is comfortable with a changing and dynamic work environment and schedule across all shifts and days

  • The ability to analyze, interpret and problem solve quickly with well-articulated action planning

  • Strong verbal/written communication skills, conflict resolution skills, and strong empathy to deliver compassionate, patient centered care
