BILH is looking for an Advanced Practice Provider (NP/PA) to join our Medically at Home program.
Job Description:
The Advanced Practitioner (APP) is responsible for providing high-quality patient care to various patient populations within the framework of Medically at Home’s model. The APP serves as the point of contact representing the Medically at Home program to the patients and family in person, collaborating with the attending physician in the Mission Control and the patient’s primary care physician (PCP). The APP performs the initial face-to-face evaluation and assessment of potential patient admissions, and upon admission, the APP will collaborate with the attending physician and the PCP to develop and implement an acute plan of care in the home.
The ideal candidate will have at least three years of APP experience and is flexible and comfortable with a changing and dynamic work environment. They will also have the ability to analyze, interpret and problem solve quickly with well-articulated action planning. Strong verbal/written communication skills, conflict resolution skills, and strong empathy to deliver compassionate, patient centered care are all a must.
