Brockton Hospital RN-Cardiology and Anticoagulation Management Services in Brockton, Massachusetts

The anticoagulation management services (AMS) RN responsibilities are to provide one on one assessment and teaching to patients on anticoagulation therapy based on established protocols under the supervision of the AMS Manager and AMS Medical Director as well as to provide support to the Cardiology office as needed under the Cardiology Practice Manager and Chief of Cardiology.

Primary duties include: Assessing and monitoring patients who are on anticoagulation under the supervision of the AMS Medical Director and AMS Manager. Provide individual INR dosing instructions based on established protocols/guidelines. Provide teaching/counseling to patient and family members regarding warfarin therapy. Refill prescriptions according to outpatient clinic policy. Perform point-of-care PT/INR testing in office. Assist with other functions within the Cardiology office as needed including rooming patients, managing tasks and triaging phone calls.

Minimum two years in ambulatory setting with telephone patient management skills. Must have previous experience providing care to patients requiring anticoagulation therapy using warfarin and/or patients with cardiac disease. Acute cardiac care experience preferred. Current licensure to practice nursing in the state of Massachusetts, BLS required. CACP certification preferred. Proficient in Allscripts and Meditech.
