Brockton Hospital Nurse Practitioner in Brockton, Massachusetts
Signature Medical Group is seeking a Nurse Practitioner or Physician Assistant to join our busy Cardiology Practice.
Responsibilities include, but are not limited to:
+ + Rounding on inpatients, meeting with patients and families, facilitating discharges and transfers, seeing and planning the care and follow-up of outpatients in the Cardiology Department.
Leading morning rounds on both the telemetry floors and CCU, stratifying cardiovascular patients early in the day to improve care and facilitate throughput.
Maintaining continuity of cardiovascular care at the hospital (follow ups) as we change consultants on a weekly basis.
Assisting with consultations
Coordinating the discharge process including scheduling outpatient follow up appointments with the appropriate cardiologist.
Supervising stress tests
Provides health education and counseling to patients
Formulates a differential diagnosis based on the history, physical examination, and diagnostic test results; establishes priorities to meet the health and medical needs of the patients, family, or community
Maintains a process for systematic follow up and evaluation of patients status; determines the effectiveness of the treatment plan with documentation of patient care outcomes; reassesses and modifies the plan as necessary to achieve patient health and medical goals
Provides referrals to specialists when clinically indicated
Assists patients in establishing linkages with services provided within the community
Develops and provides relevant patient education based upon established priorities that are consistent with evidence based criteria and clinical expertise; provides patient-sensitive teaching and learning opportunities
Provides appropriate consultation and/or referrals on behalf of the patient and family to other involved health professionals and community agencies; participates as a team member in the provision of health and medical care, interacting with professional colleagues to provide comprehensive care
Maintains accurate, legible and confidential medical records, records all pertinent data about the patient including the history and physical examination, problems, identified, interventions provided, results of care and plans for follow up and referrals.
Applies evidence based guidelines in clinical practice; maintains certification in compliance with current state law, participates in continuing education programs as required by the recertification and licensing process
Works as part of a multidisciplinary team, collaboratively practicing with other health professionals
Completes appropriate billing documentation
Assists in the extraction and submission of data as it relates to Cardiology and the MassCom trial
Performs all other duties as assigned.
Must be licensed to practice in Massachusetts.
Master's level (MSN), adult health nurse practitioner license and ANNC (American Nurses Credentialing Center) certification required or NCCPA Board Certified Physician Assistant.
Must be prescriptive eligible with 2 years’ experience as a Cardiology Nurse Practitioner.
CCU experience as an RN or NP preferable.
