Position Description/Minimum Qualifications * Position Title: Case Manager-Nurse (Field) Department: Clinical-Adult Foster Care Reports To: Program/Clinical Director Hours: TBD Field Nurse Job Responsibilities: As a Full-time Registered Nurse (FRN) you would be completing Consumer Assessments, Admission, monthly visits, and clinical review notes at the office. They are hands-off assessments in which you would be obtaining consumer demographics, physician information, and a list of medications. You would also be evaluating the consumer ADL\'s and IADL\'s along with conducting a brief cognitive screening and completing an overall summary. Skills and Qualifications: Placement, MDS Reviews, Semi-Annual Report, Progress Note, Judgment, Supervision, Informing Others, Nursing Skills, creating a Safe, Effective Environment, Home Environment, Medical Teamwork, Listening, Adult Foster Care Qualifications: - A qualified candidate will have a BS in Nursing and have a minimum of one year of experience, preferably in Adult Foster Setting. The candidate must have an active RN license in the state of practice, Massachusetts. - The ideal candidate would be punctual, organized, friendly and demonstrate exceptional communication skills.
