Boston Children’s Hospital Primary Care Center (CHPCC) is looking for an organized and motivated coordinator to support our multidisciplinary behavioral health team. This position is responsible for registry management, patient support (including scheduling, as well as facilitating and tracking referrals), and support across a spectrum of integrated behavioral health services within the primary care setting.
The Program Coordinator I will be responsible for:
Coordinating activities and daily operations of a program.
Managing a registry of patients to track referrals, ensure communication and loop closure between staff, families and outside agencies.
Scheduling appointments, making reminder phone calls and ensuring that patients and families remain up to date in care.
Supporting the team through administrative functions such as preparing paperwork for authorizations, collecting feedback from schools and acting as the liaison between the clinic and community agencies.
Interacting and maintaining liaison with staff, faculty and outside/community agencies in facilitating program objectives.
To qualify, you must have:
Associate’s with one year of relevant experience required.
Bachelor’s degree preferred.
Work requires the ability to communicate effectively both orally and in writing and providing empathy in difficult interpersonal situations.
Work requires the ability to work with diverse internal and external constituencies.
