65590BRJob Posting Title:Patient NavigatorDepartment:Ambulatory ServicesAutoReqId:65590BRStatus:Full-TimeStandard Hours per Week:40 Job Posting Category:NursingJob Posting Description:Seeking a dynamic, Spanish-speaking Patient Navigator for this unique opportunity to work as team member on a pilot project with Boston Children’s Hospital Accountable Care Organization (ACO) addressing social determinants of health for primary care families and young adults. This person will work with an interdisciplinary team in the Adolescent and Young Adult Medicine Clinic to help ensure patients and their families have stable, healthy housing; access to nutrition resources; and receive all benefits and community resources available to them.

The Patient Navigator will be responsible for:

  • Identification and assessment of patients to determine eligibility for social support interventions

  • Facilitation of referrals to community partners

  • Coordination of nutrition education and resources, including providing direct nutrition education and skills development (e.g., food budgeting) counseling and providing support to enrollment in community nutrition classes

  • Assistance with program development and establishment of program infrastructure

  • Collaboration and relationship development/management with partner community agencies, including co-management of patients

  • Providing support for acute housing and nutrition needs for patients in clinic on a rotating schedule

  • Participation in a multidisciplinary team of medical and mental health providers in a primary care medical clinic

To qualify, you must have:

  • Interest in working with multicultural patients with complex needs, in particular social determinants of health.

  • Fluency in Spanish strongly preferred.

  • Ability to observe, analyze and interpret general and technical data, (verbal and written) accurately.

  • Strong communication and interpersonal relationship skills with the ability to respond to events effectively.

  • Ability to follow research protocols and procedures and enter data.

  • Access to a private location to work from home or offsite.

  • Expertise with building community partnerships, trauma-informed care, social determinants of health, housing policy and resources, and food security.

  • Diverse candidates strongly encouraged to apply.

  • Experience in a medical setting preferred

  • High school diploma or GED required.

  • Bachelor’s degree in Human Services or related field preferred.

  • Minimum of one year of closely related experience required. Experience with clinic population or work with community resources is preferred.
