  • Providing resources and assistance to patients and families with accessing clinical and supportive care services for diabetes within Boston Children’s Hospital and in the community. Acting as a liaison with internal and community resources. May contact hospital and community resources to ensure access to services.

  • Providing supportive outreach to families of newly diagnosed diabetes patients and high-risk diabetes patients.

  • Facilitating appointment scheduling and completion of required annual screening tests for diabetes patients. May assist with scheduling appointments across specialties/disciplines. Coordinating logistics for plan adherence such as appointment reminders and arranging transportation.

  • Assisting patients with applying for insurance. Providing insurance information to care teams to determine eligibility for services.

  • Maintaining relationships for accessing community-based resources. Providing outreach and maintaining positive working relationships with providers/administrative staff, including physicians, office staff, nurses and social services staff.

  • Documenting patient care and collecting required data. Tracking outcomes and supporting strategic planning processes. Developing and maintaining data.

  • Attending and participating in care planning meetings to prepare patients for continuing care.

    To qualify, you must have:

  • High School Diploma / GED required; Bachelor’s degree in Human Services or a related field preferred.

  • Minimum of one year of closely related experience required. Previous experience with clinic population or work with community resources is preferred.

  • The ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.

  • Work required the analytical skills to resolve problems that require the use of basic scientific, mathematical or technical principles.

  • Fluency in Spanish preferred.
