The Nurse Practitioner in Primary Care will be responsible for:

Caring for patients of all ages and serving as the NP Champion for the Rainbow program. Rainbow patients are children with medical complexity whose medical home is at Boston Children’s Hospital Primary Care Center (CHPCC)

The Rainbow Nurse Practitioner will be responsible for:

Providing comprehensive assessment and care for patients.

Providing patient care coordination, health promotion and education, evaluation of acute care needs.

Working closely with people across the clinic as an integral provider and member of both rainbow team and the larger clinic.

Serving as a resource and teammate to interdisciplinary team members. Team members include but are not limited to nursing, providers, social workers, child life specialist and administrative staff.

Being involved in any rainbow quality improvement initiatives to improve care for our patients with medical complexity.

To qualify, you must have:

MSN and licensed PNP/FNP.

2 years of Family/Pediatric NP experience preferred.

Spanish speaking preferred, but not necessary.

Availability to work 24 hours per week: Monday through Friday and participate in weekend urgent care (max of 1 shift/ month).
