The BIDMC Nursing Mission is to build on a legacy of nursing excellence by caring with compassion, advancing the art & science of nursing, and advocating for the health of patients, families, and communities. The RN Case Manager (CM) facilitates the coordination of care for those patients cared for in the outpatient Hematology-Oncology and Bone Marrow Transplant (BMT) clinics at the Beth Israel Medical Center (BIDMC) in Boston. The case manager consults and collaborates in activities related to the establishment of appropriate levels of care. He/She reviews patient's medical records and participates in direct and indirect communication with care givers to ensure continued appropriate level of care. The case manager provides ongoing education to the physicians and nurses about levels of care. The RN Case Manager, performs, consults, and collaborates in activities related to the development of safe effective and efficient disposition of patients for patients cared for in the outpatient Hematology-Oncology/BMT clinics. This role is patient-focused, outcome-oriented, based on professional nursing practice guidelines and functions within a multidisciplinary practice model. Training, evaluation and competencies for this role are overseen by a Nurse Leader to the department.

Job Description:

Primary Responsibilities:

  1. Performs care coordination and home care planning for assigned patient caseload. (essential)

  2. Coordinates and organizes a timely and accurate review of each medical record for severity of illness and intensity of service to satisfy reviewing and, investigating patient history, presenting symptoms, clinical status and treatment. (essential)

  3. Utilizes the processes of assessment, planning, implementation and evaluation to create care plans for assigned patients. (essential)

  4. Facilitates multidisciplinary collaboration on a daily basis. (essential)

  5. Documents the plan of care and monitor patients on a basis determined by patient need. Evaluates the plan to insure that it is reasonable, effective, and outcome oriented. (essential)

Required Qualifications:

  1. Associate's degree in Nursing required. Bachelor's degree in Nursing preferred.

  2. License Registered Nurse required., and American Heart Associate Basic Life Support Certificate required.

  3. 3-5 years related work experience required in Oncology.

  4. Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

  5. Oral Communications: Ability to comprehend and communicate complex verbal information in English to medical center staff, patients, families and external customers.

  6. Knowledge: Ability to demonstrate in-depth knowledge of concepts, practices and policies with the ability to use them in complex varied situations.

  7. Team Work: Ability to lead collaborative teams for larger projects or groups both internal and external to the Medical Center and across functional areas. Results have implications for the management and operations of multiple areas of the organization.

  8. Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.

Age based Competencies: Employees in this job must be competent to provide patient care to the following age groups: Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 - over.

Social/Environmental Requirements:

  1. Work requires periods of close attention to work with out interruption. Concentrated effort of up to 4 hours without break may be required.

  2. Work requires constant response to changing circumstances and using new information to adjust approach and to quickly respond to new needs.

  3. Potential exposure to adverse environmental conditions

  4. Several times a week: Protective equipment required(Respirator,earplugs,mask,gloves,eyewear etc).

  5. Several times a month: Potential exposure to infectious diseases and/or airborne pathogens, Potential exposure to infectious diseases and/or bloodborne pathogens.

  6. Health Care Status: HCW1: Regular, day-to-day contact; both face-to-face and hands-on (having close contact within three feet for at least five minutes). Examples: physicians, clinical nurses, phlebotomist, medical assistants, PFT tech, and x-ray tech.- Health Care Worker Status may vary by department

Sensory Requirements:

Close work (paperwork, visual examination), Color vision/perception, Visual monotony, Visual clarity > 20 feet, Visual clarity <3 feet, Hot/Cold, Sharp/Dull, Smell, Conversation, Monitoring Equipment, Telephone, Background Noise.

Physical Requirements:

Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move through out the hospital campus

This job requires constant sitting, standing, Fine Manipulation using both hands, Keyboard use, Endurance-working up to 5-6 hours without a break,frequent bending neck.There may be occasional bending waist.Rarely there may be Pushing/Pulling using both hands.
