Description

Our nurses are the heart and soul of Fenway Health. Every day, they work tirelessly to ensure that our patients receive compassionate, high-quality care. We are currently seeking LPNs to join our team of mission-driven nurses at our outpatient community health center. As an LPN, you will provide specialized nursing support for patients and medical providers, phone support and triage, as well as general support for other clinical activities to ensure our patients receive the highest quality care.The schedule is based on clinic needs and may include covering evening and Saturday clinics on a rotating basis.

Duties:

Providing Patient Triage: the safe, effective and appropriate disposition of health-related problems

  • Administers comprehensive evaluation of patient telephone callers, “Code Blue” events, and walk-in patients, develops care plan, consults with other disciplines as needed, and implements plan including referrals

  • Advocates for patient care as nurse member of the multidisciplinary medical team

  • Care plan is communicated to patients in a timely manner

  • Documents triage encounters in medical record, documenting all appropriate activities and plans

Case Management

  • In conjunction with patient team, coordinate services to our patients to improve their care using careful assessment, planning, coordination and delivery of care, and outcome evaluation of the plan.

  • Coordinates follow up care for acutely ill patients including primary care and specialty visits, VNA, Infusion companies, and prior authorization for services

  • Coordinates preventative services for defined population including education, vaccinations, monitoring and arranging lab tests and referrals.

  • Evaluates adherence to care plan and intervenes to promote patient’s well-being

  • Documents services in medical record, documenting all appropriate activities and plan

Providing direct patient care, using nursing procedures as appropriate under nurse licensure in an outpatient clinic

  • Assists with patient care under the direction of appropriate medical provider and/or under nursing scope of practice

  • Provides nursing support to RNs, LPNs and MAs

  • Places and Maintains IV Lines

  • Administers medications vaccinations, (PO, IM, SC, PR, ID, INH, IV) per medical provider order

  • Checks supplies for nursing patient encounters and notifies manager or supply ordering designee of additional stock needed

  • Administers all medical tests and (e.g. EKG, spirometry, CLIA) and procedures (e.g. ear lavage, nebulizer) as per protocol and medical provider request

  • Works with MA and pharmacy to call in prescription refills or obtain prior authorizations

  • Assists with preventative medicine efforts (e.g.: flu, STD, vital sign checks, occupational health)

  • Coordinates follow-up care for acutely ill patients including primary care and specialty visits, VNA, Infusion companies, and prior authorization for services

  • Helps coordinate preventative services for defined population including education, vaccination, monitoring and arranging for lab tests, and referrals

  • Documents services in medical record, documenting all appropriate activities and plan

  • Obtains and delivers lab reports once reviewed by a provider

  • Assists with sample management and all necessary forms for alternative insemination

  • Collection of lab samples, including use of phlebotomy; packaging and providing for transport to lab

  • Attends to healthcare issues of specific populations

Maintains comprehensive knowledge of medical issues

  • Includes those both acute and chronic care as appropriate to an ambulatory care setting especially HIV/AIDS (i.e. opportunistic infections, medication side effects) and behavioral issues (ie medication compliance, safe sex) issues as well as other medical conditions as appropriate for nursing case management

  • Seeks out continuing education opportunities related to essential job functions

  • Completes continuing education requirements per the Massachusetts Board of Nursing regulations

  • Understands and maintains universal precautions in all clinical activities

Provides health education

  • Provides community health education

  • Provides patient education related to their medical and psychosocial needs

  • Maintains comprehensive knowledge of vaccines, medications, and other therapeutic interventions as appropriate to our outpatient clinic

  • Maintains comprehensive knowledge of FH policies, procedures and services including those of TFI

  • Maintains knowledge of community resources

  • Assists in training other employees in the medical department including orientation of new staff, volunteers, and students

Performs other related duties as required

Requirements

  • Current licensure as an LPN in the Commonwealth of Massachusetts

  • Current CPR certification

  • Ability to work harmoniously with diverse groups of individuals

Preferred Qualifications:

  • IV insertion / Phlebotomy skills

  • Supervisory experience

  • 1-year of experience in an ambulatory care setting

  • Experience with patients with HIV and electronic medical records

  • Interest in LGBT health and commitment to community health

  • Experience working with an ethnically, culturally, and racially diverse environment

Working Conditions and Physical Effort:

  • Work involves considerable exposure to unusual elements such as extreme temperatures, unpleasant odors and/or loud noises

  • Moderate physical activity Requires handling average-weight objects up to 15-20 pounds, assisting with patients and standing and/or walking for most the day

  • May work with blood or blood-borne pathogens and will require OSHA training

  • Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment

Salary Description

$28.21-$31.13 per hour ($55,000-60,703 per year)
