This individual will focus on supporting C3 Health Center Population Health Management (PHM) and Complex Care Management (CCM) staff within a designated territory as they engage in C3 Evolent driven clinical programs, quality measurement gap closure, practice transformation, risk adjustment/correct coding, and performance management support. The Delegation Oversight Manager will serve as the C3 ACO Health Center liaison to C3 and Evolent. This person must exhibit strong leadership skills, organization and communication skills, and a strong attention to detail.

Please note - This individual will be responsible for visiting C3 locations throughout the state of Massachussetts

Responsibilities:

  • Support the health center practice delegated PHM and CCM staff by analyzing performance reports and data to inform decision-making, process, and program implementation. Monitor advise delegated PHM and CCM staff and their supervisors on educational and process interventions based on health center and provider-level data, trends and identified opportunities. Inclusive of, but not limited to:

  • Advising primary care practices, health centers, nurses and other clinical staff to assist them on their conversion to value-based care

  • Identification of process improvement gaps

  • Collection and application of best practices

  • Dissemination of quality and efficiency reports

  • Providing educational and training sessions

  • Dissemination and support of Gap List Closures

  • Testing and refining of workflows that support sustainable transformation, and guide practices as they expand interventions to additional patient populations

  • Hiring, onboarding, and directing care management staff

  • Understands, monitors and drives tangible improved care management team performance on caseloads, engagement rates, outcomes, quality, population health, productivity and financial measures.

  • Works with the reporting team to develop a suite of reports for managing care team productivity and performance.

  • Assures that standards of practice and policies are in compliance with the Masshealth ACO contractual requirements and other regulatory guidelines and standards.

The Experience You’ll Need (Required):

  • Bachelor’s Degree (Degree in Nursing, Business Administration/Hospital Administration/ Public Health strongly preferred.)

  • Three to five years of supervisory or management experience overseeing care management operations.
