Coordinating the patient referrals process for care management including, but not limited to, skilled and private duty nursing, infusion and enteral therapy services, durable medical equipment, and transportation throughout the care continuum
Collaborating and delivering clear, effective communication with multidisciplinary team members to coordinate the care management activities within the assigned inpatient unit/team
Working with insurances to obtain prior authorization approvals. Planning and Prioritizing clinical scheduling and follow-up on all referral requests according to approvals
Serving as a liaison with the relevant clinical groups ensuring regular and fluid communication and developing connectivity between these areas throughout the care continuum. Coordinating with case management teams to ensure confirmation of timely delivery of medications, equipment, and supplies to patients
Assisting patients and families with inquiries, provide resources, educational literature, and direct to community partners
Supporting continuous process improvement by identifying alternatives to existing processes and participating in pilot programs as assigned
Supporting department in the implementation of initiatives through the participation of special projects as needed
To qualify, you must have:
A high school diploma / GED required. Associate’s degree preferred
Two years of relevant administrative experience in a healthcare setting
Intermediate skills in MS Office Excel, Outlook, and Word
The ability to deliver clear, effective communication via telephone, in written form, e-mail, or in person. Takes responsibility for understanding others, dealing effectively with conflicting views or issues, and mediating fair solutions
The ability to ensure information is complete and accurate; follows up with others to ensure that agreements and commitments have been fulfilled
The ability to work with and help others to accomplish objectives
This role has full time hours M-F. Position is temporary, funding will end on 12/31/22.Office/Site Location:BostonRegular, Temporary, Per Diem:Temporary
