Boston Health Care for the Homeless Program Bilingual Clinic Case Manager (Spanish/English) in Boston, Massachusetts

Hours: Hours are Monday, Wednesday and Fridays 8a-4:30pm, with Tuesdays and Thursdays 11:30a-8p

We are seeking a hard-working and solutions-oriented bilingual individual who is excited to provide legal, medical, housing, and general social support to our patients. Could this be you?

Since 1985, the mission of Boston Healthcare for the Homeless has remained to ensure unconditionally equitable and dignified access to the highest quality health care for all persons experiencing homelessness in the greater Boston area. Our dedicated clinical and non-clinical staff work hard to bring the mission alive caring for populations that suffer very high mortality rates and presents a complex burden of co-occurring medical, mental health and addictions problems which place them at great risk.

Based primarily at our 780 Albany Street facility, you will work on one of our primary care teams. In this role there is a unique fusion of clinic-based support for “Orange Team” patients and outreach efforts for the clinic’s primary care patients. You will have a special focus on supporting the needs of LEP/Immigrant patients who have additional barriers to care. You will also have protected time for supporting walk-in immigration-focused case management in addition to hours reserved for general “Orange Team” case management work and serve as an advocate and case manager for those who are ineligible for other care management programs (particularly immigrant, elderly, and/or newly homeless folks). Case management services typically include but are not limited to public benefits, housing navigation, connection to behavioral health and specialty care, food/transportation security, legal advocacy.

We are seeking an individual who is eager to support the multi-disciplinary team and is comfortable with flexibility in the workplace.

Responsibilities:

  • Provide walk-in based case management support for patients in our main clinic, working alongside a multidisciplinary team of providers, nurses, behavioral health clinicians, and case managers.

  • Provide additional support to our immigrant patients who may not have access to standard services – collaborate with the Immigrant Health Committee and Oasis immigrant clinic team to develop creative solutions to barriers to care

  • This position will include a blend of scheduled clinic sessions seeing walk-in patients, as well as flexible case management support for teams across the program.

  • Collaborate with medical providers to conduct outreach to patients.

  • Identify and develop cooperative working relationships with service providers for people experiencing homelessness, substance use disorder, and legal barriers

  • Participate in city-wide networks of case management for homeless individuals.

    Qualifications:

  • Spanish language skills required

  • Knowledge of and experience with the network of services available to homeless persons; Strong understanding of the unique needs of LEP and Immigrant patients

  • Prior case management experience a plus

  • Comfortable working with multidisciplinary team, maintaining a good rapport with nursing/medical staff, legal organizations, and other disciplines

  • Strong problem solving and communication skills (written and oral); r efined ability to communicate professionally with employees and patients, both on the phone and in-person

  • Organized, detail-oriented, and able to complete tasks in a timely manner; s elf-directed with the ability to work independently in multiple settings; flexible and adaptable to different health care delivery models

  • Proficient with Microsoft Office, entering narrative and other data into electronic medical records and other internet-based products
