About the Job
The High Risk Intervention Team (HRIT) includes the Readmission Reduction Team, SHIFT-Care Grant Team and the Moms Do Care Team. The Analyst will provide timely analysis, processing and reporting of data, and analyze, identify and work towards the identification of trends and patterns in patient behavior. The analyst will also monitor workflow within the department, produce daily and weekly reports for the team as well as reports that identify trends and areas for improvement. The Analyst is responsible for the enrolling and assigning patients to the appropriate team member. The Analyst will also be responsible for researching grant opportunities and completing proposals for submission.
The position requires strong communication and writing skills, project management skills, and exceptional organizational skills. Ability to multi task, consistently meet deadlines, and provide ongoing support to the High Risk Intervention Team department is a must.
This position will work with other groups on a daily basis. The analyst will represent the team as needed when interacting with state and local agencies, physician offices, SNFs, hospitals, and outpatient services.
This position will also provide Project Management leadership to a variety of projects of moderate scope, creating and managing to timelines and facilitating meetings.
40 hours per week, day shift.

Bachelor’s degree, plus experience with writing grants
Knowledge skills and abilities:
Action oriented, able to identify and act on new opportunities
Excellent communication (written and oral), presentation and interpersonal skills,
Program/project management skills
Competency with PC’s and MS Office suite of projects, including Visio and MS Project software
Must be competent with Excel
Must be detailed orientated, have a professional demeanor, proven problem solving abilities and the ability to work with those having wide-ranging levels of expertise
Ability to work independently and motivate diverse teams focused on outcomes
