Brockton Hospital Nurse Practitioner in Abington, Massachusetts
Signature Medical Group is seeking a Full Time, Internal Medicine Nurse Practitioner experienced in Pain Medication Management to join our practice in Abington. The Nurse Practitioner will work collaboratively with the Supervising Physician while also practicing autonomously to provide excellent patient care with an emphasis on safety and quality. The Nurse Practitioner will provide a full scope of primary care services including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance.
Primary Responsibilities: Performs patient interview, review of medical history, including all surgeries, prior testing and treatment modalities, including surgery, physical therapy and medication therapies. Performs complete physical exam, urine drug screen, pain medication assessment and pill counts. Reviews results of PEG, COMM, ORT and PHQ-9 assessments as appropriate. Develops individual treatment plan for each patient. Presents treatment plan and expected time for patient to transition from Opioid medication to other non-Opioid treatments. Establish appropriate follow-up schedule based on patients need and all current state and federal guidelines. Review PMP for all patients prior to prescribing or renewing medication orders. Perform peer to peer calls with insurance carriers in order to prevent delays in medication refills.

Reimbursement for renewal cost of licenses QUALIFICATIONS:
Graduate of an approved Nurse Practitioner program
Bachelor’s Degree required, Master’s Degree preferred
Must have current valid Massachusetts licensure, Massachusetts CSR and Federal DEA
Experience in chronic disease management and women's health conditions is preferred
Ability to work without direct supervision and practice autonomously
