With its corporate office located in Westwood, Mass., BIDCO offers physician groups and hospitals the structure to contract, share risk, and build care management systems together, with the goal of providing the highest quality care in the most cost-efficient way.Job Location: Westwood, MAReq ID: 21247BRJob Summary: The Pharmacy technician supports the Population Health Team in the development and execution of BIDCO-wide medication related strategic priorities for patients in our global payment contracts who have been identified for our Complex Care Management or Rising Risk Disease Management Program. These programs and activities include work with BIDCO providers and practices, nurse care mangers, health coaches, social workers, and collaboration with other BIDCO teams to ensure high-quality, cost-effective pharmacological care for patients in the BIDCO global payment contracts. Prepares risk stratification reports, pharmacy specific targeted intervention data, and quality metric data as it pertains to identified high-risk and rising risk populations. They will conduct pre-assessments to ensure patient is appropriate for identified interventions, and provides follow-up to ensure execution of targeted recommendations.

Essential Responsibilities:

  • Prepares and distributes multiple monthly risk stratification reports per targeted interventions for both programs to both internal and delegated staff.

  • Uses population health analytics and electronic medical records to collect pre-assessment data including but not limited to; current medication list, drug allergies, most recent lab data, specialty providers involved in care, refill history claims/pharmacy contact), last office visit and upcoming appointments.

  • Populates pre-assessments templates and forwards to nurse care manager, clinical pharmacist or health coach per workflow for review.

  • Adheres to high risk and rising risk program elements specific to pharmacy optimization to prevent disease progression/complications as well as reduce unnecessary pharmacy costs, and close gaps in care.

  • Completes all required pre and post-assessment documentation in order to track success.

Required Qualifications:

  • Associate's degree required.

  • Registration Registered Pharmacy Tech required., and Certificate 1 Certfied Pharmacy Technician required.

  • 1-3 years related work experience required.

  • Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases.

Preferred Qualifications:

  • Experience with pharmacy claims analysis, health plan policies, prior authorization requests, and Medicare Part D plans preferred.

  • 5+ years as a Nationally Certified Technician in an ambulatory or managed care setting preferred.
