Position

Community Paramedic

SCHEDULE:

  • Monday – Friday

  • No weekends no holidays

QUALIFICATIONS:

  • BLS CPR certified

  • Minimum of 5 years of EMS experience with at least 3 years as a paramedic.

  • 3 years as a paramedic supervisor preferred.

  • Training, CQI, marketing and/or public communication experience and proficiency.

Job Requirements:

  • Examines, screens, treats, and coordinates health services for patients.

  • Conducts post-hospital release follow-up care including, but not limited to, monitoring medication, dressing changes, and checking vital signs.

  • Observes, records, and reports to the physician, patient’s conditions and reaction to medications, treatments, and significant incidents.

  • Conducts patient education, including diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessment, injury evaluation, geriatric frailty visits, and nutrition.

  • Administers patient care consistent with clinical guidelines and provider orders; Coordinates appointments and follow-up with physicians, hospital staff, and community health center staff.

  • Develops and completes appropriate reports and templates for the mobile care program and attends meetings as requested and available.

  • Operates a Baystate Health vehicle in accordance with Baystate Health policies. Other duties as assigned.

The base pay range shown above considers the wide range of factors that are considered in making compensation decisions including knowledge/skills; relevant experience and training; education/certifications/licensure; and other business and organizational factors. This base pay range does not include our comprehensive benefits package and any incentive payments that may be applicable to this role.
