The Intake Manager facilitates and promotes the acceptance of appropriate patients to the home health care setting for all three sites-Central, East, West and the Hospice and Palliative programs. Oversees seamless referral process from all referral sources. Ensures effective communication with referral and payor sources. Works in cooperation with all departments to ensure detailed, accurate, appropriate and timely clinical and payor information is documented to facilitate timely delivery of services and billing of services provided. Acts as a positive and professional role model for team members.

  • Promotes operating excellence, customer satisfaction and efficient and effective patient care by ensuring the completion of all aspects of the referral management process, according to department guidelines

  • Responsible for evaluation of all complex referrals to Baystate Home Health programs which includes obtaining and processing all information needed for successful admission to service. Processes and documents referrals that are not accepted for admission, including specific reason for non-acceptance, according to department policy and procedure. Vigilant in the secure and release of Face to Face documentation to initiate care

  • Provides day to day supervision of the referral management process, supporting an environment that assures team members demonstrate competency in their roles and provide excellent service to both internal and external customers

  • Ensures adequate staffing with properly trained personnel; ensures that staff orientation is complete and appropriate; makes recommendations regarding employment, promotions, transfers and terminations, and monitors staff productivity.

  • Maintains oversight of seamless Intake/Referral process by responding to issues promptly and works to provide superior customer service. Interprets statistical information to analyze referral data for timeliness and compliance.

  • Provides clinical coverage as needed for weekdays, weekends and holidays to ensure efficient and effective management of referrals, 7 days/week.


What You Will Need:

  • Registered and Licensed Nurse in the State of Massachusetts.

  • BSN and clinical or operational supervisory experience strongly preferred

  • Minimum of 3 years of home care nursing experience preferred.

  • Excellent communication skills, oral, written, critical thinking, problem solving; ability to prioritize work tasks for self and others

  • Strong commitment to customer service

  • Ability to work independently with attention to detail.

  • Demonstrated ability to be a team player and work within an interdisciplinary team.

Demonstrates knowledge base of home care regulations and standards, payor specific information, and Baystate Home Health programs and is a resource to the Intake Department

Applicants must possess a valid driver's license recognized by the employee's state of residence and satisfactory finding from their Motor Vehicle history report as a contingency of employment.
