Responsibilities

The HIV Ambulatory Nurse collaboratively leads the development and delivery of high quality patient care including coordinating care and services for patients with Hepatitis C, HIV infectious disease and or addictions.

Will function as a community liaison and will be expected to have the capacity to offer the following components of medical case management: medical care coordination, phone check-in, social services coordination, adherence support, HIV/STD/HCV risk reduction counseling, addiction services, benefits counseling, coordination of home delivered meals if required, as well as coordination of other services as needed (e.g., transportation, peer support, housing, etc).

Qualifications

  • Massachusetts RN licensure and CPR

  • 3 year of nursing experience preferred

  • Must be capable of working in a fast-paced clinical team environment and be able to demonstrate cultural competence working with a diverse patient population wit may include socio-economic issues and LGBTQ client population.

  • Excellent assessment and triage skills as well as strong interpersonal, organizational and communication skills are essential.

  • Preferred candidates will have strong knowledge of HIV addictions and or infectious disease, have clinical nursing experience as well as care management for similar patient population and have expereince with educating patients on preventative health h o.

  • Bilingual (English/Spanish) is strongly preferred
