General Statement of Duties:To provide supports necessary to enable persons with developmental disabilities to develop and maintain skills that will foster personal growth and independence. These duties must be carried out in a manner that

respects the rights of individuals, is driven by individual choice, and based on the concept of community inclusion.

Responsibilities:

  1. Provide direct service in both basic care (toileting, hand and face washing, eating, etc.) and programmatic (fitness, gross motor, communication, sensory, functional living, leisure, self-preservation, etc.) areas in the community and within the program.

  2. Actively participate in program development, maintaining activity manual in program area as directed.

  3. Fulfill case management and advocate responsibilities for assigned caseload.

  4. Provide feedback to program manager and other treatment team members regarding consumer progress or concerns in a timely manner.

  5. Insure that group and leisure times are spent on age appropriate, preferred activities while maintaining a skill development focus.

  6. Participate in supervision, staff meetings, team meetings, ISP meetings, etc. as directed.

  7. Implement behavior plans as required.

  8. Organize and participate in group and individual activities, addressing objectives and guidelines established in the individual service plans.

  9. Perform duties in a team approach.

  10. Maintain knowledge of developments with program.

  11. Provide physical support to consumers having physical challenges such as lifting and transferring consumers as needed.

  12. Record necessary data, including but not limited to, communication notebook, ABC sheets, and data on ISP objectives.

  13. Complete monthly progress notes, incident reports, communication books, behavior sheets, ISP documentation, etc. as required.

  14. Participate in ISP semi-annual reviews and other meetings as required.

  15. Maintain current and accurate documentation (written plans and data sheets) in program areas relating to objectives being addressed.

  16. Ensure consumer safety and a safe environment in compliance with agency and program standards.

  17. Intervene in crisis situations that may include physical restraints.

  18. Perform duties to reflect agency/program policies and philosophy.

  19. Execute duties to reflect reasonable safety standards. Standard/universal precautions must be utilized and training obtained in areas that constitute risk.

  20. Perform other related work duties as needed or as assigned by supervisor.

Requirements
