Must be a Licensed Practical Nurse in the state of of Massachusetts. At least 2 years working in an acute or chronic care setting with individuals with acquired brain injury or similar disabilities is required. Must have a valid driver's license in state

of residence and satisfactory driving record.

This position is responsible for floating to all day programs including Bridgewater, Easton, Lakeville, Middleboro, West Roxbury, Quincy, Brockton and Stoughton.

DescriptionGeneral Statement of Duties: Identify health related needs, monitor medical status and work with staff, physicians and allied health professionals to formulate, coordinate and implement medical treatment plans for all consumers.

Responsibilities:

  1. Administer medications and treatments as prescribed, ensuring adherence to CARF, DDS, Medicaid and DPH regulations.

  2. Provide individual and/or group consumer and staff training in relevant areas (e.g., health hygiene, safety, diet, etc.).

  3. Monitor consumer lunch breaks as assigned.

  4. Maintain an assigned caseload of consumers and act as case manager as directed.

  5. Provide direct support to consumers as needed.

  6. Provide assistance at other day habilitation programs as needed.

  7. Complete written reports, assessments, etc. as required by the program and its funding and certification sources in a clear, concise and professional manner. This includes, but is not limited to, monthly nursing progress notes, ISP and annual review documentation.

  8. Maintain medication books, appropriate sections of consumer files, etc. to ensure program compliance with all regulations established by DDS, DPH, CARF and Medicaid.

  9. Ensure annual updates of physical examinations, emergency treatment forms, doctors’orders etc. in order to meet established guidelines.

  10. Advise the program manager and staff of any medical problems that may hinder a consumer's progress or that affect participation in program.

  11. Ensure that needed medications are dispensed by staff on community outings.

  12. Ensure adequate safety and medical supplies are on hand (e.g., gloves, gowns, etc.).

  13. Perform other related work duties as needed or as assigned by supervisor or designee.

  14. Provide supervision of daily operations in absence of supervisor or designee, as requested.

  15. Participate in supervision, ISP review and all other required meetings as assigned.

  16. Perform duties to reflect program and agency policies and procedures.

  17. Perform duties with a team approach.

  18. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.

  19. Provide physical support, assistance with lifting and transferring consumers who have physical challenges as needed.

Requirements• Licensed Nurse in the Commonwealth of Massachusetts.

• At least 2 years working in an acute or chronic care setting with individuals with acquired brain injury or similar disabilities required..

• Knowledge of rehabilitation activities, ISP development, CARF regulations, DDS philosophy, and behavior management experience desired.

• Must possess a valid driver’s license in state of residence and satisfactory driving record

• A demonstrated ability to make appropriate, accurate judgments including handling of emergency situations, and the mental and physical ability to successfully perform job duties and responsibilities which include, but are not limited to, providing physical

support to consumers having physical challenges such as lifting and transferring persons served as needed.

• Basic computer literacy, including the ability to record data, use electronic time and attendance software and receive and send electronic mail, as needed, is required.

• A demonstrated ability to make appropriate, accurate judgments including handling of emergency situations, and the mental and physical ability to successfully perform job duties and responsibilities which include, but are not limited to, providing physical

support to consumers having physical challenges such as lifting and transferring persons served as needed.

• The ability to communicate effectively, both verbally and in writing with persons served, families, service providers and case managers.

• Demonstrated ability to set limits, to solve problems independently, and to appropriately protect confidential information.

• Demonstrated ability to monitor and report on a participant’s progress, challenges and status verbally or in writing as required.

• Demonstrated ability to interpret vocational rehabilitation evaluations and individual service plans and to conduct situational assessments.
