Job Details:Community health nurse to manage a caseload of patients receiving skilled nursing services in their homes. Provides direct patient care, care coordination and case management as primary nurse within a multidisciplinary team.Provides comprehensive health care to patients directly in their homes. Assesses physical and psychosocial needs of the patient. Determines the needs of the patient and provides clinical interventions based on ongoing assessments and MD orders. Develops a follow-up care plan with each visit. Assesses and coordinates multidisciplinary services in the home and/or by case conference. Assesses individual patient/family learning needs, style and limitations and makes adjustments for the delivery of information. Assesses clinical findings reporting significant findings to the appropriate physician for intervention and follow-up. Assessment of need for continuing services.Must have current professional nursing licensure in the state of Massachusetts. A minimum of one year of acute medical/surgical nursing experience is required. Two or three years of acute medical/surgical experience and/or community health experience preferred. Must have a driver's license and current CPR certification.No home care experience necessary.
