Department Description: The BIDHC-Jamaica Plain primary care practice is a culturally diverse practice that offers a full spectrum of treatment with a particular focus on preventive care. Our Jamaica Plain primary care physicians (PCPs) remain dedicated to fulfilling patients' needs including same-day appointments for ill patients with an urgent need.Job Location: Jamaica Plain, MAReq ID: 21692BRJob Summary: Performs the duties of a Licensed Practical Nurse and implements the plan of clinical care for the ambulatory setting according to practice guidelines. Functions as clinical resource for the Practice Assistants and Medical Assistants within the practice.
Department Specific Job Responsibilities:

Performs clinical competencies and tasks within the scope of the license as a practical nurse. Provides continuity of care to designated patients by following the plan of care. Assists with prescription refill requests in collaboration with physicians and call in orders to pharmacy.
Assists in patient education activities in collaboration with the physicians, NP's, RN's and other professionals in the practice. Maintains safety of patients who have been identified at risk for preventable injury by the implementation of nursing safety interventions or the use of protective devices. Demonstrates technical competency when performing nursing procedures and techniques.
Maintains positive relationships with all patients, families and visitors, as well as co-workers, physicians and all BIDHC/APG personnel. Coordinates and follows through on a wide variety of patient care issues as directed by the RNs, NP's or physicians. Contributes to decisions/formulating recommendations as it relates to patient care issues or general practice issues.
Perform other duties as needed and provide coverage when others are out or to assist with patient flow. Perform accurate and ongoing observations of the patient and report this information to the RN, NP or physician. Administers medications and intravenous fluids accurately, safely, and in accordance with established policies and procedures.
Communicates the status and pertinent information about designated patients accurately and concisely. Records on approved BIDHC/APG forms and systems and in accordance with established policies and procedures for medications and treatments administered and information gathered. Maintains the confidentiality of patient information.
Vocational or Technical training required in Diploma in Nursing from a professional accredited Practical Program.
License required: Licensed Practicing Nurse, and Certificate 1 required: Basic Life Support
1-3 years related work experience required.
Fluent in English and Spanish, at a level that ensures accurate and understandable interpretation and translation
and Medical terminology.
- Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.
- Three years experience in a clinical setting, preferably within a primary care setting.
Written Communications: Ability to communicate clearly and effectively in written English with internal and external customers.
Oral Communications: Ability to comprehend and converse in English to communicate effectively with medical center staff, patients, families and external customers.
Knowledge: Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.
Team Work: Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.
Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.
