Position Description/Minimum Qualifications POSITION SUMMARY: Utilize the Nursing process to provide optimum quality and continuity of nursing care. Demonstrate the core nursing competencies established by the Agency to ensure effective and efficient care on an ongoing basis. Oversees client?s satisfaction and needs by coordinating care that is safe, timely, effective, efficient, equitable, and client-centered. Liaison for clinical communication, resource management and promote quality and cost- effective interventions and outcomes. Help clients achieve wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, service facilitation, and use of evidence-based guidelines or standards RESPONSIBILITIES: Promote focused on improving clients clinical, function, emotional and psychosocial status. Oversee the collaborative process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet an individual?s health needs through communication and available resources to promote quality cost-effective outcomes Lead activities to improve clients? health and promoting wellness and autonomy through advocacy, communication, education, identification of service resources, and facilitation of service. Partner with PCP and advocate for clients/support systems. Achieve quality outcomes for GH clients and commit to the appropriate use of resources and empowerment of clients in a manner that is supportive and objective. Ensure services are offered according to the clients? benefits as stipulated in their health insurance plans. Vital participants in the care coordination team who empower people to understand and access quality, efficient health care. Collaborate with professionals from within or across health care organization, such as provider, employer payer and community agent Promote optimum level of wellness, self-care management, and functional capability: These stakeholders include the clients themselves, their support systems, the healthcare delivery systems including the providers of care, the employers, and the various payor sources. Embrace and adopt to cultural competency, based on: cultural beliefs, values, and needs of clients/support systems and in collaboration with all services providers (both healthcare professional and paraprofessionals) EDUCATION & QUALIFICATION REQUIREMENTS: Graduate of an accredited School of Nursing, College or University. Must possess at least one of the following combinations of education & experience: Baccalaureate Degree in Nursing (BSN) or a health or human services field; OR Associate?s Degree or Diploma in Nursing plus one year of certified home health care nursing or medical surgical nursing experience preferred Currently licensed and registered to practice with the State Board of Nursing Home Health experience preferred. Current CPR certification. Demonstrate leadership, organization and communication expertise. Excellent verbal and written communication skills.
