Bear Mountain Health Care Registered Dietitian for Long Term Care Center in East Longmeadow Full Time in East Longmeadow, Massachusetts

We are in search of a Registered Massachusetts Licenced Dietitian. This is a full time position. Must have Long Term Care experience.

The Dietitian is primarily responsible for assessment and evaluation of residents’ nutritional needs in skilled nursing facilities and/ or assisted living facilities providing counseling to residents and family members to promote health and wellness and control of disease.

Complete nutritional initial, quarterly, annual and significant change reviews on residents according to federal and state guidelines.

Review monthly and weekly weights to determine those residents which have had a significant change.

Complete nutritional reviews monthly on high risk residents (significant weight loss/ gain, pressure ulcer, hemodialysis, and tube fed).

Assess nutritional needs, diet restrictions and current health plans in order to develop and implement dietary care plans and provides nutritional counseling as needed.

Attends and participates in care plan conferences, Standards of Care, and other clinical meetings as directed.

Consult with physicians and other health care personnel (Nurse Practitioners, Dialysis Dietitians, etc) to determine diet restrictions and nutritional needs of residents.

Assist in coordination of nutrition care services with the Dietary manager.

Attends and participates in morning meeting/ stand up to facilitate communication with the team.

Monitor food service operations to ensure conformance to nutritional, safety, sanitation and quality standards, as well as, state and federal regulations.

Monitor food control systems such as food temperatures, portion control, preparation methods, garnishment and presentation of food in order to ensure that food is prepared and presented in an acceptable manner. Inspect diet trays for conformance to physician’s diet orders prior to delivery.

Review, approve and assist with implementing preplanned cycle menus that confirming to company policies, state and federal regulations.

Completes monthly sanitation inspections including tray line audits. If benchmark is not achieved weekly inspections will be completed.

Complete test trays to evaluate nutritive value, flavor, appearance, and quality meals ensuring proper temperature and food form is followed to meet individual residents’ needs.

Advise dietary managers and Administrator on sanitation, safety procedures, menu development, budgeting, and planning to assist with the establishment, operation and evaluation of food service department and nutrition programs.

Meets with Administrator reporting on procedures and activities of the department.

Reviews the results of the sanitation inspections and tray line audits and other reports with the Administrator, Director of Nursing and the Dietary Manager.

Monitor food service operations to ensure conformance to nutritional, safety, sanitation and quality standards.

Monitor food cost procedures to assist the Dietary Manager with maintaining food budgets.

Monitor supplement use to assist the Director of Nursing and Dietary Manager with maintaining supplement budgets

MINIMUM QUALIFICATIONS:

Ability to meet all health, compliance, and competency requirements

Ability to read, write, and speak English; and communicate effectively verbally and in writing.

One to three years of professional experience and/or training; or equivalent combination of education and experience.

Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Ability to perform all assigned tasks in accordance with the Facility’s established policies and procedures, and as directed by supervisors.

Ability to complete required orientation as directed by the facility.

Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

Has successfully completed state required background check and is eligible to work in the facility.

Knowledge and understanding of regulatory process i.e. Federal, and/ or State Regulations.

Knowledge and understanding of the RAI Process.

Familiar with the Nutrition Care Process, utilizes PES statements administering Medical Nutrition Therapy to residents.

Skill to apply principles of personnel administration to interviewing, selection, supervision of employees, scheduling and monitoring departmental activities.

Computer literacy skills (Microsoft Outlook, Microsoft Office, Internet)

Registered Dietitian or completion of approved baccalaureate degree with major studies in Food and Nutrition and eligible for RD exam.
