Department Description: Population Health-WestwoodJob Location: Boston, MAReq ID: 21672BRJob Summary: Participate in the development and execution of BIDCO-wide clinical pharmacy management programs and activities as they relate to strategic priorities and global payment contracts. These programs and activities include their work with BIDCO providers and practices to ensure high-quality, cost-effective pharmacological care for patients in the BIDCO global payment contracts and providing continuous education on pharmacy related matters to our provider community. Pharmacy specific support targeted for complex high-risk patients and rising risk populations are conducted either directly or in conjunction with providers, registered nurse care managers (RNCM), health coaches, and in collaboration with other BIDCO teams as needed, with goals of preventing unnecessary hospitalizations and ER visits, thereby reducing the patient's overall medical expense and inappropriate utilization. The clinical pharmacist also supports the education of pharmacy-related quality metrics and provides recommendations to enhance quality scores by reducing gaps in care.

Essential Responsibilities:

  • Executes clinical pharmacy disease management programs and activities as they relate to strategic priorities for risk patients in our global payment contracts.

  • Provide drug therapy recommendation to assure safe, accurate, rational and cost-effective use of medications in key disease states to providers, nurse care managers, and allied health professionals. Collaborates with nurse care managers on high risk care management cases specific to medication management and disease management program initiatives as identified.

  • Conduct medication reviews and provide recommendations for identified high risk/rising risk patients; reducing unnecessary or duplicative medications, streamlining regimens to enhance adherence, identifying gaps in care, recommending therapy escalation for patients not meeting clinical goals, and reducing drug-drug interactions

  • Provide unbiased clinical drug information support to BIDCO providers, NCM, and allied health professionals.

  • Develop initiatives for quality metrics with pharmacy component for patients not meeting goals and coordinate efforts with BIDCO IT/EHR teams. Performs identified measures of success for each program domain that pertain to adherence to best practices, clinical outcome measures, and patient satisfaction.

Required Qualifications:

  • Bachelor's degree in Pharmacy required. Doctoral degree preferred.

  • License Pharmacist required.

  • 3-5 years related work experience required.

  • Travel in the eastern Massachusetts area required.

  • Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases.

Preferred Qualifications:

  • Experience in a Managed Care environment preferred.
