The Case Management Associate (CATCR) will be responsible for:Collaborating and communicating with multidisciplinary team members around pre-treatment planning, referral processing and service set up.Communicating with patients and/or family members as necessary to assist with information, provide literature, and direct to community resources.Acting as a liaison with the relevant clinical groups (both inpatient and ambulatory) and developing and maintaining strong relationships between these areas.Communicating with clinics, community resources, agencies, vendors and families around patients’ transitional care.Communicating with insurance companies to obtain not only prior authorization approvals for medications, but also supplies, equipment and ambulance transportation.Ordering durable medical equipment and faxing corresponding orders and necessary documentation as identified by the clinical team.Providing resource information, literature and direction to community resources and maintaining community resources database.Providing data entry into hospital Electronic Medical Records (EMR).Coordinating special projects as needed.Developing and maintaining internal database that supports patient care.To qualify for this position you must have:A Bachelor’s Degree; degree in a health care related field is preferred.Minimum of 2 years administrative health care experience in coordinating care. Medical Terminology skills, a plus.
